I understand and voluntarily agree that:

1. I have not knowingly withheld any information that might adversely affect my chances for employment and that the answers given by me are true and complete to the best of my knowledge.  I certify that I have personally completed this application.  I understand that any misrepresentation or omission of material fact in this application or document used to secure employment shall be grounds for rejection of this application or for immediate discharge if I am employed, regardless of the time elapsed before discovery.

2. I understand that any offer of employment I may receive from Gilroy's Kitchen + Pub + Patio is contingent upon my successful completion of the Company's total pre-employment screening process, including the Company's receiving references that it considers satisfactory.

3. I understand that nothing contained in this application or conveyed during any interview that may be granted is intended to create an employment contract between Gilroy's and me.  I understand and agree that if I am employed, my employment is for no definite or determinable period and may be terminated at any time, with or without prior notice or cause, at the option of myself or CABCo, and that no promises or representations contrary to the foregoing are binding on the Company unless made in writing and signed by the Managing Partner of Gilroy's Kitchen + Pub + Patio (Gilroy's).

4. I authorize and request Gilroy's to thoroughly investigate my references, work record, education and other matters related to my suitability for employment and, further, authorize my former employers and those individuals I have listed as references to disclose to Gilroy's any and all letters, reports and other information related to my work record, including statement of the reason for the termination of my employment, work performance, abilities and other qualities pertinent to my qualifications for employment without giving me prior notice of such disclosure.  In addition, I hereby release Gilroy's, my former employers and all other persons, corporations, partnerships and associations from any and all claims, demands or liabilities arising out of or in any way related to such investigation or disclosure.
